I hope that you and your friends will enjoy a magical night in Hollywood! Guests may only make reservations if they have an invitation from a member. Since I am a member, I would like to offer you an invitation.
Even though you have an invitation to make a reservation, you will still need to pay for the door charge, your dinner or brunch, and parking. The evening door charges are $25 per person on Monday - Thursday and $35 per person on Friday - Sunday. Children are not able to visit the club at night, but may go to the Saturday or Sunday brunch. The brunch door charges are $25 for adults and $20 for children ten years or under. Infants are free of charge. Dinner entrees range from $38 - $50. Valet parking is $15.
The dress code for dinner is "business attire or evening wear that is conservative, formal, and elegant." The dress code for the Saturday and Sunday brunch is "conservative, country club casual, or formal party-wear." Thus, please state below if you would like an invitation for the more formal dinner hours or an invitation for the more laid-back brunch hours.
Whether you choose to visit the club during the dinner or brunch hours, you will still be able to see the constantly changing performances before and/or after your meal. You will receive a ticket for the main performance at the end of your dinner or brunch.

Be sure to type the "code word" which I gave you in the "reason why the invitation was granted" box. This is my way of making sure that I am sending an invitation to a particular person rather than just anybody who found this form on the Internet.
